Hello resident Campers!

I have attached the medical forms that will be required to attend resident camp.  Parts A, B and C will be necessary.  Part D is NOT required to be filled out as this is not a high adventure event.  A and B are filled out by you and part C by the physican.

Each adult and scout will be required to get a physical and have the doctor fill out part C.  I strongly encourge everyone to get the physical sooner rather than later.  Last minute scheduling can be difficult to obtain and no one will be allowed to camp without this getting done.

I have also enclosed the Epi pens/inhaler forms for anyone who has food allergies and/or asthma.

Completed forms need to be turned in to me as I will be turning them in to the camp and keeping track of everyone.

As we get closer to summer, I will be checking in with all of you and passing information along as it comes in.
